I hold at least a 10th-grade certificate.

I hold an accredited training certificate or a childcare-provider practice certificate.

I declare that neither I nor any adult resident in the home has a conviction for a felony or misdemeanor involving moral turpitude, public decency, child abuse, or addiction.
